2024 - 2025 Information & Documents

All School District of Philadelphia students, as per SDP policy, are required to be immunized in order to attend District schools and programs. Pennsylvania law requires all students (K-12) be vaccinated while attending school virtually or in-person. Due to safety reasons, students are not permitted to carry medication (OTC medication, vitamins, asthma pumps, Neosporin, etc.) at any time. If your child requires medication during the school day, you and your child’s physician must complete and submit a MED-1 form to the CWH school nurse before medication can be administered.

The Annual Student Emergency and Medical Information form (S865) needs to be completed every new school year by the student’s guardian. It can be completed online here: https://www.philasd.org/studenthealth/#forms or a paper copy can be given to the school nurse.
